Our Mission

The Mission of The Home Possible Project is to create and provide residential options, services, and support designed to meet the needs of adults with developmental disabilities and similar disabling conditions. We are dedicated to fostering growth and independence through vocational, social and recreational opportunities.

Our Goal

Our Goal is to proactively address an orderly transition of care for adults with developmental and similar disabilities from their aging family members to a stable, reliable and caring environment that is sustainable over a lifetime. We plan to provide each individual with surroundings that are conducive to long term friendships and a sense of well-being and self-worth.

Our Core Values

At the heart of our organization are our Core Values: Dignity, Respect, Inclusiveness, Equality, Diversity, Integrity, Excellence, Independence, Empowerment, Sustainability, and Community. These values guide everything we do as we work towards creating a more inclusive and supportive community.

Our Approach

We are following the Visions for Creative Housing Solutions Model

“it has proven to promote self-determination, autonomy, and independence by providing tailored support that helps residents grow and thrive. Living among a caring circle of friends, with individualized wrap-around support services, allows each person to lead an engaged and fulfilled life, as a contributing member of our community.”

Join us in creating a future where everyone has a place to call home and the support they need to thrive.

Tara is 48 and has lived in Gilford, NH most of her life with her family. She has one younger brother, a nephew, and a cat. Tara is very social and outgoing, and enjoys dancing, shopping, camping and cooking with others. She also works part-time at Lakes Region Community Services.

Tara has lived with her parents in Gilford and they have always been her primary caregivers. Tara’s father had a serious illness and recently passed away.
